  • Case management, diagnosis and treatment services for children and young adults under age 21, who are medically, financially and residentially eligible. Physical and occupational therapy services for eligible children at school-based sites.

Child and Adolescent Health
  • Program provides referrals for no-cost well-child and adolescent health examinations and immunizations; case management of children with identified health conditions; case management and environmental investigations for children with lead poisoning; health education presentations and materials on various child health topics; school-based dental disease education and prevention services, and fee-for-service tobacco education for schools.

Child Health and Disability Prevention (CHDP)
